When to Consider a Referral
On Standard HRT But Still Symptomatic
Testosterone, estrogen, and progesterone are optimized. Labs look appropriate. The patient still doesn't feel well. This almost always points to thyroid dysfunction, adrenal dysregulation, or a functional medicine issue that wasn't part of the original workup.
Thyroid Involvement Suspected or Confirmed
Patients who need desiccated thyroid extract (DTE), compounded T3/T4 protocols, or a full thyroid antibody workup fall consistently outside standard hormone replacement scope. If thyroid is in the picture, PHC is the appropriate next step.
Adrenal Dysfunction Is in the Picture
Adrenal fatigue, subclinical insufficiency, and HPA axis dysregulation present with profound fatigue, poor stress tolerance, and orthostatic symptoms. These patients are frequently misattributed to hormone deficiency alone and don't respond to standard replacement.
PCOS or PMDD Is Part of the Presentation
Both conditions involve hormonal, metabolic, and inflammatory components that require evaluation beyond what standard hormone replacement addresses. PHC manages these as integrated clinical presentations, not isolated symptom clusters.
