NAD+ Isn't a Longevity Switch. It's Fuel for Your Cells' Repair Crew.

Maybe it was an IV, a patch, or a wellness ad promising to "reverse cellular aging." Whatever the format, you felt a real lift for a few days, then watched it fade back to baseline, and now you're wondering whether NAD+ actually does anything, or whether you just paid for an expensive placebo.

Neither answer is quite right. NAD+ decline is real, measurable, and mechanistically tied to a lot of what patients in their 40s, 50s, and beyond describe as "just getting older": low energy, slower recovery, foggier thinking, a body that doesn't repair itself the way it used to. But the idea that NAD+ is a switch you flip, something that instantly restores younger cellular function once levels go back up, misunderstands what NAD+ actually does and why it declines in the first place.

What NAD+ Actually Does

NAD+, short for nicotinamide adenine dinucleotide, is not a hormone and it isn't a supplement in the way people usually mean that word. It's a coenzyme, a molecule your cells require to run hundreds of biochemical reactions, most importantly the ones that convert food into usable energy inside your mitochondria. Every time a cell burns glucose or fat for fuel, NAD+ is shuttling electrons through that process.

NAD+ also fuels a separate but related job: it's the required cofactor for a family of proteins called sirtuins. Sirtuins regulate DNA repair, inflammation control, and the genes that govern metabolic efficiency. Without adequate NAD+, sirtuins can't do their job, even if every other input, exercise, sleep, nutrition, is optimized. A landmark review in the journal npj Aging and Mechanisms of Disease laid out this relationship clearly: NAD+ and sirtuins function as a coordinated pair, and age-related decline in one degrades the function of the other (Imai & Guarente, npj Aging Mech Dis, 2016, PMID: 28721271).

So yes, NAD+ matters. It's fuel for literally hundreds of repair and energy-production processes running in every cell in your body, all day, every day.

Why NAD+ Declines, and Why "Aging" Is an Incomplete Answer

Most explanations stop at "NAD+ falls as you age." That's true, but it treats the decline as a passive leak, like a battery slowly draining. The more complete picture is more useful clinically, because it points to something you can actually act on.

The Enzyme Nobody Mentions: CD38

CD38 is an enzyme sitting on the surface of certain immune cells. Its normal job involves breaking down NAD+ as part of routine immune signaling, in small, manageable amounts. The problem is that CD38 activity isn't constant. It rises with chronic, low-grade inflammation, and when it rises, it consumes NAD+ directly, faster than your cells can replace it. Research published in Cell Metabolism identified CD38 as a major driver of age-related NAD+ decline (Camacho-Pereira et al., Cell Metab, 2016, PMID: 27304511).

More recent work has filled in why CD38 activity climbs in the first place. Research from the Buck Institute, published in Nature Metabolism, found that senescent cells, aged cells that stop dividing but don't die and instead linger while releasing inflammatory signals, actively recruit immune cells called macrophages into tissue and push them to express high levels of CD38 (Covarrubias et al., Nat Metab, 2020, PMID: 33199924). The result is a feed-forward loop: senescent cells recruit CD38-expressing macrophages, those macrophages burn through NAD+, and the resulting cellular stress creates more senescent cells. NAD+ isn't just fading passively with age. Something upstream is actively consuming it.

This is the piece that gets left out of most NAD+ marketing. If your NAD+ is low because an inflammatory process like this is still active, simply adding more NAD+ doesn't fix the underlying drain. You're refueling a tank with a hole in it.

NAD+ decline is real. But raising the number without addressing what's consuming it faster than it can be replaced is fueling, not fixing.

What Repletion Can and Can't Do

This isn't a case against NAD+ support. A well-designed randomized trial in healthy middle-aged and older adults showed that chronic oral supplementation with nicotinamide riboside, a NAD+ precursor, at a dose of 1,000 mg daily for six weeks, was well tolerated and raised measured NAD+ levels in white blood cells by roughly 60 percent compared to placebo (Martens et al., Nat Commun, 2018, PMID: 29599478). That's a real, reproducible effect on the number.

What that trial doesn't show, and what no NAD+ trial to date has convincingly shown, is that raising the number alone reliably resolves the symptoms patients are chasing when they seek this out: sustained energy, sharper cognition, faster recovery. Some patients on NAD+ support do feel meaningfully better. Others feel a short-lived lift that fades. The difference often comes down to whether the upstream driver, the inflammatory or metabolic process consuming NAD+ in the first place, was ever addressed.

Where the Inflammatory Drivers Usually Hide

In practice, the patients who see the most durable benefit from NAD+ support are the ones where it's layered onto a system that's already being brought back into balance, not used as a substitute for that work. Chronic low-grade inflammation has several common drivers in this population: insulin resistance, unresolved gut or metabolic dysfunction, and poor sleep among them.

The Hormone Piece Almost Nobody Checks

There's another driver that's just as common and gets missed far more often, because it rarely shows up on a standard workup. Thyroid hormone, estrogen, progesterone, and testosterone all influence inflammatory tone and mitochondrial efficiency directly, not peripherally.

A patient with suboptimal thyroid function or declining sex hormones is often running a low-grade inflammatory state before a single symptom ever gets connected to hormones at all. Their labs may sit inside a "normal" reference range while their actual physiology is working against them. Optimizing that piece doesn't just improve energy on its own, it also reduces the inflammatory drain that's consuming NAD+ in the background.

This is why sequencing matters more than stacking. Identifying and addressing the upstream driver first, then supporting NAD+ status, tends to produce a more durable result than adding NAD+ on top of a system that's still actively inflamed.

A Composite Patient

A patient in their early 50s came in after two rounds of NAD+ IV therapy at a med spa. Each infusion produced a noticeable lift for three days, followed by a return to the same fatigue and brain fog that brought them in originally. Basic labs from their primary care doctor had come back "normal."

A broader workup told a different story: elevated inflammatory markers, insulin resistance not yet severe enough to be labeled diabetes, and thyroid and sex hormone levels sitting at the low end of the reference range, technically normal, functionally insufficient. Who Should Look Into This, and What to Expect

If you've tried an NAD+ product, whether an infusion, a patch, or an oral precursor, and gotten a short-lived response that didn't stick, that's a signal worth paying attention to, not a reason to write off NAD+ entirely. The more useful next question is what's driving the drain: inflammatory markers, metabolic labs, and a comprehensive hormone panel that goes beyond a basic TSH or single testosterone value.

A realistic timeline runs over weeks to a few months, not days. Addressing an upstream inflammatory or hormonal driver takes longer to show up in how you feel than an IV drip does, but it tends to hold. Monitoring should include repeat inflammatory and metabolic markers alongside a honest conversation about symptoms, not just a single before-and-after energy rating.

One honest caveat: NAD+-boosting therapies aren't appropriate for everyone. Because NAD+-dependent pathways are also used by rapidly dividing cells, anyone with a current or recent history of cancer should discuss NAD+ support specifically with their physician before starting it.
